Insurance Plans

We want all of our patients to feel welcomed, safe, and that they've received exceptional care. During your initial visit, expect your provider to review your personal and medical history and perform an exam to address any current medical issues. We may ask you to return for a prevention-focused physical if completing that is not possible at your first visit due to focus on current problems. If blood-work is required we draw samples on site. For a shorter time in the waiting room, please complete the e-check in link that was sent to you via email or text.

 

We are In-Network and accepting new patients with most insurance plans. It is your responsibility to contact your insurance to verify participation with our supervising physician Dr. Chad Costley. If you have a HMO Plan that requires you to select a PCP it is your responsibility to call and have the PCP updated to Dr. Chad Costley. This must be effective on the date of your visit.

We also welcome self-pay patients. Please visit our Self-Pay Guide for more information.

We do not accept the following insurance plans. Please bring your current insurance card with you to every visit.

New Patient Process

To prioritize accessibility we try to make our new patient process as simple as possible. Please review these basic steps and then head over to our Patient Guide for more detailed information about on-boarding new patients and helping you with the administrative side of receiving care.

Step 1: Review our Policies & FAQ

Prior to booking an appointment, please understand our Practice Policies including Office Visits vs Wellness Exams, Accepted Insurance, Fees & Billing, Lab work, What to Expect, and other pertinent patient info.

It is your responsibility to contact your insurance to verify participation with our supervising physician Dr. Chad Costley. If you have a HMO Plan that requires you to select a PCP it is your responsibility to call and have the PCP updated to Dr. Chad Costley. This must be effective on the date of your visit.

Step 2: New Patient Scheduling

We are currently accepting new patients! To book an appointment, please use the following link to schedule your new patient visit: https://consumer.scheduling.athena.io?locationId=9773-1. Please call the office directly for urgent issues or gender affirming care.

When scheduling, please include the following information and complete necessary registration steps:

  • reason for visit

  • Insurance carrier name, Member ID # and Group #

  • Please be sure to have your medical records faxed to 844-246-7292 prior to your first visit

  • Register for the patient portal and complete self check in at least one week prior to appointment (see Step 3 for further details)

Step 3: Complete e-Check in

All New Patients will receive a portal registration invite and an e-Check in link prior to the appointment. Please complete this promptly upon receipt to ensure that all of your information is correct in our system, to sign off on all electronic forms, and to confirm your appointment.

We ask that you complete this registration process at least one week prior to your appointment. In order to provide a comprehensive and efficient first visit, your appointment will be cancelled if this information is not received 3 days prior to your appointment.
