We want all of our patients to feel welcomed, safe, and that they've received exceptional care. During your initial visit, expect your provider to review your personal and medical history and perform an exam to address any current medical issues. We may ask you to return for a prevention-focused physical if completing that is not possible at your first visit due to focus on current problems. If blood-work is required we draw samples on site.

For a shorter time in the waiting room, please fill out the New Patient Form and email it to us before your visit.

Step 1: Review our Policies & FAQ

Prior to booking an appointment, please understand our Practice Policies including Office Visits, Accepted Insurance, Fees & Billing, Lab work, What to Expect, and other pertinent patient info.

Step 2: Call to Make an Appointment

We are currently accepting new patients! To book an appointment, please call our office and our team will help schedule an appointment with the appropriate practitioner at a convenient time.

Step 3: Complete New Patient Packet

All New Patients are asked to complete a New Patient Packet and fax or email back to our office prior to their first visit. This helps streamline your visit so we can focus on providing quality care.
