Welcome to Ponce Primary Care, a full-service primary care practice offering comprehensive prevention and disease management care to adult and teenage patients.  

Founded in 2008 and conveniently located in downtown Decatur, we've developed a strong reputation for the quality of care we provide and being accessible to our patients.

Our mission is to help our patients live healthy lives by providing accessible, comprehensive, personalized and coordinated primary care.


Chad D. Costley, MD, Owner/Founder

Dr. Costley is the founder and owner of Ponce Primary Care. His medical practice focuses on preventive and chronic disease management of adult patients. He has been named one of Atlanta’s Top Physicians by Atlanta Magazine each of the past three years. In addition to his medical career, Dr. Costley holds a MBA degree from Emory University and leads an investment group focused on supporting early-stage technology companies. He has served on a number of small business and non-profit boards of directors. He enjoys fly fishing, distance running, studying history and traveling with his wife LeighAnn and their two sons.

Dr. Emily Walker, MD

Dr. Walker joined Ponce Primary Care in 2014 after completing residency training at the University of Connecticut where she served as chief resident.  Her professional interests include management of acute and chronic illness, diet and health behaviors, and office-based procedures, such as skin biopsy and joint injections.  She is interested in women's health and provides care to many transgender teenagers and adults.  She lives with her wife who is also a family physician and her two young children.  She enjoys being active outside, cooking and sharing meals with family and friends.

Kara Brown, NP-C

Kara worked at Mckinney’s Apothecary in downtown Decatur before joining Ponce Primary Care in 2012.  With a passion for preventive medicine, wellness, and acute illness care, Kara also enjoys medical mission work having spent time in Costa Rica, Mexico, Honduras, and Bahamas. In her spare time Kara runs a foot clinic for homeless men and women at the Healing Community Center. She is also an avid rock climber, runner and soccer enthusiast.

Kara is married with a young daughter and has two dogs, a cat, and a turtle who also keep her busy.

Kristina Lemene, DNP , NP-C

Kristina has worked in family practice since 2005 and has a passion for preventive health and wellness with a special focus on managing diabetes and cardiovascular disease. She believes it is important to have a strong working relationship with patients to help patients achieve their health goals. 

Kristina lives in Atlanta with her husband and three children. She also serves as part time clinical nursing faculty at the University of North Georgia.

Tara Noorani, FNP-C

Tara grew up in Southern California where she discovered her passion for community health while working as a caseworker for UCLA Mobile Clinic – a street-side medical clinic designed to support people experiencing homelessness in Los Angeles County.

Tara provides primary care to teens and adults for preventative medicine, wellness exams, hormone replacement therapy and HIV prevention. Her passion is to ensure LGBTQ+ identities are made visible within healthcare; therefore, she works to integrate gender affirming care and a trauma-informed approach into her nursing practice. She hopes to empower each person to make informed decisions about their care and achieve their individual wellness goals. 

Outside of work, Tara enjoys hiking, playing basketball and discovering new restaurants around Atlanta.
