Welcome to Ponce Primary Care, a full-service primary care practice offering comprehensive prevention and disease management care to adult and teenage patients.  

Founded in 2008 and conveniently located in downtown Decatur, we've developed a strong reputation for the quality of care we provide and being accessible to our patients.

Our mission is to help our patients live healthy lives by providing accessible, comprehensive, personalized and coordinated primary care.

 

Chad D. Costley, MD, Owner/Founder

Dr. Costley is the founder and co-owner of Ponce Primary Care and remains active in a clinical leadership role with the practice as Physician Lead. He earned his MD degree at the University of Michigan where he also completed his post-graduate residency training. He is board-certified in Family Medicine and has 20+ years of clinical experience in the breadth of primary care medicine. Dr. Costley also holds a MBA from Emory University and serves as the CEO of BlueWillow Biologics, a small biotechnology company developing intranasal vaccines. While Dr. Costley is not currently accepting new patients, he is engaged daily with Ponce Primary Care's provider team in the care of our patients. He is married with two adult children and enjoys fly fishing, reading and working to improve his Spanish.

Kristina Lemene, DNP , FNP-BC (she/her), Co-Owner

Kristina has worked in family practice since 2005 and started practicing at Ponce Primary Care in 2016. She has a passion for preventive health and wellness with a special interest in managing diabetes and cardiovascular disease. She believes it is important to provide compassionate and evidence based care to build a strong working relationship with patients to best identify and help achieve their health goals. 

Kristina lives in Atlanta with her husband and three active school-aged children. She enjoys being outdoors and traveling with her family.

Tara Noorani, FNP-C (she/they)

Tara grew up in Southern California where she discovered her passion for community health while working as a caseworker for UCLA Mobile Clinic – a street-side medical clinic designed to support people experiencing homelessness in Los Angeles County.

Tara provides primary care to teens and adults for preventative medicine, wellness exams, hormone replacement therapy and HIV prevention. Her passion is to ensure LGBTQ+ identities are made visible within healthcare; therefore, she works to integrate gender affirming care and a trauma-informed approach into her nursing practice. She hopes to empower each person to make informed decisions about their care and achieve their individual wellness goals. 

Outside of work, Tara enjoys hiking, playing basketball and discovering new restaurants around Atlanta.

Brooke Littleton, FNP-C (she/her)

Brooke is a family nurse practitioner who joined Ponce Primary Care in 2020. She completed her Bachelor’s and Master’s of science in nursing at the University of North Georgia. Brooke’s medical interests include pediatrics, women’s health, chronic disease management, and preventative medicine. Having been a registered nurse for many years on a cardiac intensive care unit she also has a strong passion for managing cardiovascular disease. She enjoys learning, and strives to stay up to date with advances in medicine to ensure her patients maintain a healthy lifestyle. Brooke lives in Atlanta with her husband and enjoys outdoor activities, reading, and weekends in the North Georgia mountains.

Savannah C. Johnson, DNP, FNP- BC (she/her)

Savannah is originally from Tennessee where she attended The University of Tennessee and completed her Bachelor’s Degree in Nursing and Doctorate in Nursing Practice. Her doctoral studies focused on increasing colorectal cancer screenings in minority communities. Prior to becoming a board-certified Family Nurse Practitioner, she worked in the intensive care unit of a level 1 trauma center.

 Savannah is passionate about preventative medicine, women’s health, and chronic disease management. She takes great pride in providing holistic, compassionate, and high-quality care to her patients.  Savannah lives in Atlanta with her husband. In her free time, she enjoys exploring the city, spending time with family, and traveling.

Amy Maresca, FNP-C (she/her)

Amy is a native of the Atlanta area. She completed her Bachelor’s Degree in Nursing at Mercer University in 2007 and earned her Master’s of Science in Nursing from Georgia State University in 2013. She has worked as a family nurse practitioner in primary care, hospital medicine, and an orthopedic spine center. She places a high priority on working with her patients to help them achieve their health goals, with a focus on preventative medicine and chronic disease management. She also places a high priority in addressing underlying stress and anxiety that may be impacting her patients’ health in a negative way.

Amy lives in the Atlanta area with her husband, three daughters, and two dogs. She enjoys riding horses whenever possible as well as bike riding, hiking and generally just playing outside with her family. Amy also holds a Bachelor’s of Science Degree with a focus in Animal Science from the University of Georgia.

Alison English, FNP-C

Alison (Ali) has been a board certified family nurse practitioner since 2013. She has lived and worked in the Decatur area her entire life. She completed her Bachelor's and Master's of Science in Nursing from Georgia State University.

The past 5 years she has been working in internal medicine and enjoys working with patients to optimize and prevent illness with a combination of lifestyle modifications and medications when necessary. She is passionate about chronic disease management including diabetes, obesity, and cardiovascular disease. She recognizes that everyone's health and circumstances are unique and regularly takes a shared decision making approach with regards to treatment decisions.

Ali lives in Decatur, with her husband and two kids. When she is not working she is often cheering on the sidelines for one of her kids, gardening, or reading by a pool if she can.

Cydni Martin, PA-C

Cydni is a board-certified Physician Assistant (PA). She was born and raised in Kennesaw, Georgia. Cydni earned her Bachelor of Science in Public Health from Georgia Southern University in 2014 and later went on to attend Morehouse School of Medicine where she earned her Master of Science in Physician Assistant Studies. She is a member of the American Academy of Physician Associates/Assistants (AAPA) and Alpha Kappa Alpha Sorority, Inc. 

As a PA, Cydni is passionate about honing in on lifestyle medicine to prevent and manage chronic diseases. She works with her patients to set realistic goals while also providing evidence-based care. She has special interest in preventative medicine, weight loss management, & mental health. 

In her free time, Cydni enjoys playing tennis, traveling, and trying new local restaurants. She also loves to spend time with family, friends & her Boston Terrier, Nelly.
